You are very welcome to self-refer for assessment. Please use my main contact details, which you can find here. Please bring any relevant medical reports with you. Also note that if you have not had an assessment done by an ear, nose and throat doctor (ENT), I may ask you to do this.
If you are a medical referrer, please email me with a report if possible. If you need to send a paper copy, please contact me for the address.
